SELF, JUANITA C., 84, of Louisville, passed away September 17, 2011.
Mrs. Self was a native of Braxton County, WV. She was preceded in death by her husband, Melvin Self.
Survivors include her loving children, Linda Goeing (Dennis), Charles Self (Betty), Cheryl Phillips (Paul) and Melvin Leslie Self; siblings, Carl, Connie, Kay, Kendell, Billy and Dennis; grandchildren, David, Holly, Russ and Pam; and great-grandchildren, Aaron, Logan, Ashley and Lindsey.
A Celebration of Life Service will be held 2 p.m. Tuesday at Evergreen Funeral Home. Visitation will be at the funeral home from 3-8 p.m. Monday.
